Drug-resistant "superbugs" are spreading at an alarming rate among critically ill patients in Bangladesh's Intensive Care Units (ICUs), with the mortality rate for newly infected individuals soaring above 90 percent. The grim reality of hospital-acquired infections was revealed on Saturday (July 25) in a press release by the International Centre for Diarrhoeal Disease Research, Bangladesh (icddr,b), following two comprehensive scientific studies.
The first study analyzed data from 373 critically ill adult patients admitted to the ICU of a specialized public hospital in Dhaka between July 2023 and February 2024. Results revealed that 53 percent of patients who had no drug-resistant bacteria upon admission acquired the deadly pathogens while undergoing treatment in the ICU. A parallel study conducted in the same hospital's Neonatal Intensive Care Unit (NICU) among 363 newborns uncovered similarly alarming figures.
Published in renowned international medical journals Microbiology Spectrum and Antimicrobial Resistance & Infection Control, the studies identified four primary Gram-negative bacteria driving hospital-acquired transmissions: E. coli, Klebsiella pneumoniae, Acinetobacter baumannii, and Pseudomonas aeruginosa. These pathogens rapidly render conventional and potent antibiotics ineffective, posing severe health hazards.
The research highlighted that 48.5 percent of admitted adult ICU patients harbored these drug-resistant bacteria in their guts—a state known as colonization. Notably, out of 198 uncolonized patients upon entry, 105 became colonized after coming into contact with infectious agents inside the ICU. Utilizing advanced Whole-Genome Sequencing (WGS), researchers demonstrated how these latent microbes escalate into fatal infections.
Experts attributed this perilous situation to systemic flaws in hospital infection control, acute shortages of sterilized medical equipment, and overcrowding in critical care units.
"Through regular monitoring, laboratory testing, and genome sequencing of ICU patients, we have clearly mapped how drug-resistant bacteria spread and how dormant pathogens in the human body transform into life-threatening infections," stated Dr. Gazi Md. Salahuddin Mamun, Assistant Scientist at icddr,b and lead author of the study.
Emphasizing the urgency of the crisis, Dr. Fahmida Chowdhury, Principal Investigator and Head of the Antimicrobial Resistance (AMR) Research Unit at icddr,b, stressed the need for systemic reform. "A hospital should be a sanctuary for healing, not a source of new infections or mortality. To protect vulnerable patients, it is critical to enforce rigorous sanitation, promote responsible antibiotic stewardship, and overhaul infection prevention protocols in sensitive facilities like ICUs," she said.
Medical experts warn that without immediate national policy interventions on strict antibiotic regulations and sterile hospital environments, intensive care units across the country will become increasingly hazardous for critical patients.
